Organization Description

Siyan Care Corporation is a mental health and behavioral health care organization based in Santa Rosa, California. It provides outpatient psychiatric and therapeutic services to people of various ages — including adolescents, adults, and older adults — primarily across Sonoma and neighboring North Bay counties.

Program Information

Provide direct clinical care: Conduct intakes, develop treatment plans, deliver individual therapy, and co-facilitate IOP groups using evidence-based practices under supervision. Complete timely, compliant documentation: Write assessments, treatment plans, progress notes, risk assessments, and discharge summaries that meet medical necessity and payer requirements. Manage risk and follow clinical protocols: Conduct suicide risk assessments, develop safety plans, consult with supervisors on high-risk cases, and follow crisis procedures. Participate in supervision and team collaboration: Attend individual and group supervision, present cases, coordinate with psychiatry and other providers, and maintain ethical and professional standards.
