Organization Description

Central Valley PACE (Program of All-Inclusive Care for the Elderly) in Modesto and Merced, CA, is a senior wellness center that provides comprehensive medical and long-term care to help older adults live independently in their homes and communities, serving as an alternative to nursing homes.

Program Information

Students would participant in the Multidisciplinary Team's initial assessments, care planning and period re-assessments that are minimally every 6 months. Assessments are completed prior to the scheduled team meeting. Students would document in the electronic medical record, refer participants and families as indicated to appropriate community agencies and acts as a liaison with such organizations and as an advocate for participants. Students would consult with and advise staff members as to the relationships or social, emotional and cultural factors to heath and medical care.
