Organization Description

Serenity Health Services (SERENITY) is a community-based organization committed to improving health, stability, and overall well-being for vulnerable individuals and families. Our mission is to provide restorative and transformative services to individuals, families, and communities that inspire hope and independence. SERENITY offers Enhanced Care Management, Community Supports, mild-to-moderate behavioral health services, care coordination, outreach, and family-centered support. Our purpose is to reduce barriers, strengthen access to care, promote self-sufficiency, and build meaningful partnerships that help individuals and communities thrive.

Program Information

Student activities and responsibilities may include supporting outreach, ECM and Behavioral Health intakes, case coordination, documentation, resource navigation, and family engagement. Students may assist with identifying client needs, completing individualized care plans, connecting individuals and families to community resources, and participating in care coordination or case staffing meetings. Students may also conduct client visits with a Lead Care Manager or Community Health Worker to observe and support person-centered services in the community. Depending on placement, students may assist with Behavioral Health, Enhanced Care Management, Community Supports, youth programming, and other community-based services. Students may also attend partnership and community stakeholder meetings to gain exposure to collaborative service planning, referral pathways, and community engagement strategies. Students are expected to demonstrate professionalism, confidentiality, cultural humility, ethical practice, strong communication, and a commitment to serving vulnerable individuals and families with dignity, compassion, and respect.

Health & Safety

No additional onboarding requirements prior to placement. However, they will participate in the agency onboarding process after placement.
