Organization Description

Stanislaus County has a variety of departments the assist with vital needs of the communities within the county.

Program Information

Students will learn how to do home assessments and case management for family caregivers and frail older adults throughout Stanislaus County. They will gain exposure and knowledge of county services and providers, in addition to collaborating with other peers who work with older adults. Students will become familiar with homevisits to various parts of the county, charting and data entry of assessments and reassessments as well as making telephone contact to clients and other community resources, as needed.
