Obtain a Student Service Learning Request Form and follow the instructions on the form (also here for your reference).

INSTRUCTIONS - For consideration, request forms must be fully complete and include all requested documents.

• Submit a cover letter. Cover letters are an opportunity for your first impression. Include information such as reason/purpose for your request, the school information, class enrollment, the number of student service learning hours to be completed, an instructor’s contact information etc.
• Attach proof of class enrollment.
• Submit a copy of the time record log form to be used for recording completed student service learning. - INDICATE THAT THIS IS COMPLETED ELECTRONICALLY FOR CSUMB.
• Present a copy of the Instructor/Student service learning agreement, if applicable.
• Valid ID
• Vaccination records for MMR, Pertussis (Tdap, must be current) and influenza (optional).
• The completed request form and attached documents may be submitted to a Center Director or the Human Resources Department office located at 1274 Broadway Ave, Seaside, CA 93955 or hr@earlydevelopmentservices.com.
