Opportunity Summary 

With the post-hospital team, the student intern will have the opportunity to perform the following:

 

-Review and process referrals

-Safety plan with families and clients

-Coordinate continuity of care for clients

-Program intake orientation groups

-Follow up with clients and families as needed

-Administrative tasks
