Case Manager InternAt — Project Hope Alliance

Opportunity Summary 

Our site-based program provides unique exposure to individual case management and programming within a social service setting. Students will work with highly experienced case managers and program managers across several student age ranges. Interns directly assist clients by providing basic needs assistance to our clients and offering support during monthly program events. Interns also assist in managing and tracking the initial intake-screening process, research available community resources for clients, as well as provide support to program staff on an as-need basis. Program Interns largely have the opportunity to work alongside case management staff by shadowing case managers to learn how to engage with clients, how to provide case management services and assist in developing and implementing improvements in our services. 

Tasks:

Intern Duties and Responsibilities
• Research available community resources and help link families/students to resources
• Assist with the planning and organizing of events and field trips
• Pack pantry and drop off pantry for students/families
• Shadow Case Managers to learn how to engage clients, build rapport, and provide case management services
• Assist with phone duties by returning call from people seeking resources and connecting them to appropriate resources
• Input Needs Assessment data onto an excel document
• Promote Project Hope Alliance services by attending community fairs to educate the community on the services provided
• Attend and participate in Case Discussions, team meetings, and staff trainings
• Assist Case Managers with Group Work (i.e print documents, call slips, set up and clean up for group work presentation)
• Provide administrative assistance for Case Managers (i.e create enrollment packets, scan & file documents, update data in Salesforce)
• Provide support to Program Staff on an as-need basis

Training:

Students will complete an onboarding general employer training to learn about the organization's mission, values. Students will receive some training on trauma informed care and case management 101. Students will shadow experienced case managers. Students will have weekly one-on-one meeting with Supervisor.

Learning Outcome:

By the end of the academic internship, the student will learn about how to build rapport and engage clients, they will learn about different community resources and how to provide case management services, they will learn about trauma informed care, they will learn the definition of students who are experiencing homelessness and how McKinney Vento Act defines homeless youth and what services are provided to youth who meet the definition of Mckinney Vento.
