Attends to the personal care of adults with developmental disabilities living in a licensed residential home.

Assists and participates in the planning, coordination and delivery of daily living activities; including outings, to facilitate development of living skills for the residents and help them achieve a higher level of independence.

Tasks:

Provides personal care for residents which may include assistance with eating, personal hygiene, showering, restroom assistance, and preparing for bedtime.

Assists and/or prepares meals in compliance with dietary restrictions; assists individuals to eat, including those who may
require assistance with G-tube feeding.

Assists in the administration and monitoring of medications.

Assists in care and cleaning of residential home, which includes housekeeping, & laundering of residents’ clothing.

Conducts daily activities to assist in the development of the residents’ living skills, consistent with their Individual Program Plan (IPP) and Individual Support Plan (ISP) goals.

Provides continuous active treatment and engagement.

Completes all required reports and documentation on a daily basis.

Attends all necessary staff meetings and completes all required mandatory trainings.

May use company provided vehicle to transport individuals to health care appointments and other various locations.

Participates in recreational activities in a variety of settings.

Performs other duties as assigned.

Training:

NAE - New Associate Experience (Remote Orientation)

Within the first (40) hours of providing individual services in the home, all new direct care staff shall complete an on-site orientation.

Monthly in-service meetings.

Scheduled Relias trainings.

Learning Outcome:

Facilitate development of living skills to neurodivergent individuals.

Implement Individual Service and Program Plans.

Gain familiarity with charting, medication administration, and positive behavior support.
