Opportunity Summary 

The Direct Care Staff is under the Direct Supervision and guidance of the Administrator/QIDP and R.N. The DCS play the most important single role in the overall development and care of each individual client. The primary duty of the DCS is to follow each client’s ISP Plan, Health Plan and provide Active Treatment.

Opportunity Learning Outcomes 

By the end of the academic internship, the student will be able to: 1. Promote client independence in all critical skills development. This means student will assist as needed to ensure proper grooming and hygiene are completed daily. 2. Demonstrate knowledge of each person’s individual ISP Objectives and implement as stated in the plans. Plans are developed by Occupational, Speech, Physical, and Behavior Therapists, Dietitians, and Nurses and student will also serve as a member of the Interdisciplinary Team and provide direct input regarding the clients’ ISP and assist the consultants with their assessments and evaluations as needed. 3. Be responsible for client safety and health, which means following the Health Care Plans. This includes the proper passing of medication (after proper certification) and reporting all health related issues to the R.N. 4. Follow the proper menu and recipes. This means knowing & following the specific diet for each client, as ordered by the Primary Physician. 5. Provide proper and safe physical techniques while assisting clients with self-care needs such as toileting and shower needs. This includes being familiar with adaptive equipment such as wheelchairs, hoyer lifts, shower chairs, etc.

Opportunity Training 

The intern will demonstrate their learning outcomes by: 1. Complete daily documentation in Therap and General Event Reports (GERs) as needed. This means documenting the daily notes per shift, behavioral episodes/incidents and all training objectives. 2. Be responsible for client safety and health, which means following the Health Care Plans. This includes the proper passing of medication (after proper certification) and reporting all health related issues to the R.N. 3. Provide proper and safe physical techniques while assisting clients with self-care needs such as toileting and shower needs. This includes being familiar with adaptive equipment such as wheelchairs, hoyer lifts, shower chairs, etc. 4. Perform assigned household cleaning duties. Clients can be involved in simple chores such as dusting, cleaning dishes, laundry and vacuuming. 5. Document and communicate all maintenance issues and Complete Shift Communication forms at the end of each shift. 6. Follow the proper menu and recipes. This means knowing & following the specific diet for each client, as ordered by the Primary Physician. 7. Teaching clients skills in health, occupational, speech, and physical therapist objectives implemented in their individual service plan. Student will practice the techniques in those fields.
