Opportunity Summary
Work with an experienced team of case managers providing case management services to older adults, adults with disabilities, and their caregivers. Learn how to develop a person centered case plan and help the consumer achieve their goals. Develop relationships with community partners to provide resources for the consumer. Have an opportunity to work with our 5 Aging and Disability Resource Connections to offer a "no wrong door" system to the people we serve.
