Case Manager – Substance Use Disorder Program
The Case Manager provides care coordination and supportive services to clients enrolled in a substance use disorder treatment program in accordance with LA County SAPC and program guidelines. Responsibilities include assisting clients with accessing community resources, coordinating services with external providers, supporting treatment engagement, and documenting services in compliance with SAPC standards. The Case Manager collaborates with the clinical team to address housing, employment, medical, legal, and social service needs while promoting recovery, stability, and successful treatment outcomes.
Tasks:
Conduct client intakes, gather relevant demographic, medical, social, and substance use history.
Assist in developing individualized care plans addressing housing, employment, medical, legal, and recovery needs.
Coordinate services with internal treatment teams and external providers, including community resources and agencies.
Participate in case review meetings and multidisciplinary team discussions.
Document case management activities, including progress notes, referrals, and follow-up, in compliance with SAPC standards.
Observe and practice motivational interviewing, crisis intervention, and relapse prevention techniques under supervision.
Assist clients with scheduling appointments, completing forms, and accessing recovery support services.
Participate in professional development and training workshops to enhance understanding of recovery-oriented case management.
Training:
Students will receive a structured orientation and training, including an initial employer orientation on policies, HIPAA/TPO compliance, and SAPC documentation standards; weekly one-on-one mentor meetings; job shadowing of experienced case managers; workshops on substance use disorder treatment, recovery support, and relapse prevention; provision of work samples; and guidance on assigned tasks to contextualize learning. Students will also review relevant literature and complete practical exercises to reinforce skills in recovery-oriented, compliant case management.
