WHAT YOU’LL DO

\n

The Journey Health Community Health Worker (CHW) is a key member of the care team serving as a bridge between the patient, the community, and clinical and social resources. As a member of our care team, you will build trusting relationships with patients and their caregivers, removes barriers preventing patients from accessing needed care, and supports patients in achieving their care plan goals.

\n

Responsibilities

\n

• Conduct telephonic and/or face-to-face outreach with patients to identify social determinants of health impacting patient's health and overall wellness

\n

• Coordinate with the patient, caregivers, and other members of the care team as necessary to ensure the care plan is implemented based on patient-directed priorities and goals

\n

• Utilize coaching, motivational interviewing, and other evidence-based techniques to support patients in achieving their goals

\n

• Identify barriers to achieving targeted clinical or social outcomes, and engage the care team to revise the care plan when necessary

\n

• Work with care teams to develop transition plans for patients transitioning from acute or post-acute settings to the home

\n

• Utilize technology and digital resources to monitor ongoing care activities

\n

• Facilitate, track, and manage referrals to key treatments and services

\n

• Document patient-related activities and interactions in electronic medical record or appropriate documentation platform

\n

• Collaborate with Journey Health’s care management team which includes physicians, nurses, pharmacists, therapists, social workers, community health workers, and medical assistants

\n

• Perform other related activities as assigned

WHAT YOU NEED

\n

Required Qualifications

\n

• Community Health Worker Certification

\n

• Experience in a healthcare setting, community-based organization, social work environment, case management role, or any combination of education and relevant lived experience

\n
