We are responsible for compliance for Title III C funding. The Acuity screenings, assessments, for our nutrition programs are extensive. In home visits are done with each senior that is onboarded to the program. The student will be conducting interviews and completing Office on Aging forms. We are audited annually by the County for all funded services. Attention to details, accuracy, and thoroughness is essential. The engagement of our care management program will include in home visits, wth senior s and often including family members identifying their needs, connecting them with the needed resources, establishing a 3 month care plan one on one with seniors. These are home bound seniors who have been determined to be the highest of risk. Often mental cognitive, wellness will be presented. Denial from family members and extended services brought in. we are mandated reporters so APS and Police well checks occur. Student will learn how and when they are needed. Lunch program, activities and programs will expose the student to the senior who is still able to socialize, has mobility and generally not experiencing the issues of isolation and anxiety of our home delivered participants..Student will see the full aging cycle and may even have a participant they meet in one program be transferred to another program.

Training includes a general overview of our organization, mission and scope of services. Resources available that we reference with other partnerships who cover areas outside our services. Training on phone assessments. Complete training on assessment questions. In home visits are done with trained Age Well staff.

During the internship, the student will be doing in home visits with seniors, conducting initial intakes (with trained staff) assessments, by phone and in person(with trained staff), assisting with senior programs and activities. Assisting with the lunch programs.Matching seniors with volunteers for Friendly Visitors. Assisting with production of a services video which will include interviewing participants receiving home delivered meals and the lunch program.
