San Antonio Regional Hospital
The Community Health Improvement Program, through a dedicated interdisciplinary team, focuses on appropriate patient identification, prevention, education, evidence-based disease management and exemplary treatment of individuals with chronic conditions. A novel stratification scale that assesses socioeconomic status, education, mental health history, adherence potential, psychological stress factors and support is utilized along with standardized clinical assessments to individualize evidence-based clinical strategies based upon each person’s respective needs. Comprehensive patient report cards include individual assessments of hospitalizations and outpatient encounters, as well as longitudinal pre-post measures of health status and patient outcomes.
