Organization Description

Health Projects Center (HPC), established in 1980 as a 503(c) non-profit organization, offers health-related and social programs to health professionals, older adults and family caregivers. Health Projects Center exists to address the health and human needs of individuals, families and communities of California's Central Coast by developing and implementing high quality programs and strategies.

Health Projects Center serves the Monterey Bay region of California, and hosts the Del Mar Caregiver Resource Center, Multipurpose Senior Services Program (MSSP), and the Central Coast Area Health Education Center.

Services of MSSP program include helping low income elderly to remain as independent as possible in their own homes.

Del Mar Caregiver Resource center provides support for family caregivers of loved ones with cognitive impairment.

Program Information

MSSP offers free social work and nursing services to medically isolated, frail and under-insured older adults who are at risk for premature institutionalization or loss of independence. We provide care management services to assist clients with daily living needs so that they may live healthy, active lives.

Social work intern responsibilities will vary depending on assignment, educational level and program (BSW vs MSW) and may include the following duties:

SC- Client phone contacts, monthly case management client contacts, clinical documentation, reassessments, complete service requests, warm hand offs and other community agency referrals for seniors in Santa Cruz County, purchased services, creating authorizations, updating care plans, attending weekly and monthly supervision with other staff including MSWs and RNs.

• Conducting in-home in-depth assessments (Initial Psychosocial Assessments/IPSA and/or Annual Reassessments) covering psychosocial, rehabilitation, and environmental concerns

• Calling clients on a monthly basis (per grid schedule) to review their care plan, issues, services, and concerns

• Identifying client service needs and requests. This may include, but is not limited to, prescription requests from medical providers, follow up with pharmacy and/or other collateral agencies, purchase orders (One Time Only and Ongoing authorizations), and service follow up that items have been received. • Coordinating and attending quarterly home visits (per grid schedule)

• Providing education and referrals to client, caregiver and/or family members (in-house referrals and/or community referrals) • Participating in developing care plans, coordinating services and conducting follow up and monitoring of client’s needs and care plan;

• Identifying and developing support systems for the client

• Electronic case recording, and reporting

 

HPC Policy on Intern/Student Driving: HPC staff shall not drive with students/interns in their personal car. If students/interns wish to participate in field based experiences, this is encouraged, but students will be responsible for transportation to and from these field based activities. If the student will drive themselves, the student must first provide HPC with proof of CA driving license and insurance and participate in our DMV monitoring program. The student may also arrange for other transportation (friend, bus, Uber etc.). Students will not drive with either HPC staff or HPC client in student car.
