Description of Organization: Drake House is a board and care facility for older adults who experience SPMI (severe and persistent mental illness). Our organization provides mental health services on site including dispensing medication, providing structured, therapeutic group activities, linking clients with needed services including health care, counseling and psychiatry and providing clients with tools needed to maintain themselves at this level of care as well as prepare them for more independent living settings.

Major Responsibilities:
 Provide and/or arrange for clients to receive counseling, support and assistance in managing daily living/independent living skills, managing psychiatric symptoms, and supports clients in working towards their treatment plan goals and objectives.
 Document clients' progress towards goals in the form of progress notes.
 Learn and practice administering psychosocial assessments, mental status exams, collaborating with, and developing, clients' annual treatment plans.
 Assists clients in identifying and accessing medical, educational, social, prevocational, rehabilitative or other community services.
 Assists with the day-to-day operations of the facility including data entry, prompting clients for group activities, sometimes providing transportation to clients for medical appointments, and keeping office and client activity areas organized.
 Responsible and mandated to report any "suspected abuse of dependent adults and elders" directly to law enforcement, Ombudsman/Advocate and Department of Social Services, Community Care Licensing ombudsman and any other agency as appropriate. Mandated to comply with suspected abuse reporting policies and procedures.
 Maintain confidentiality when working with clients.
 Maintain professionalism when communicating with all stakeholders (e.g. clients, families, county employees, psychiatrists, physicians, ombudsman).

Qualifications:
 Must be 21 years or older and working towards a Master's degree.
 Prefer previous coursework in Personality, Clinical, Community and Abnormal Psychology.
 Must be fingerprinted and cleared by the Department of Justice and/or FBI (Live scan).
 Must submit TB test that has been completed within the previous 6 months.
 Must be computer literate and proficient with Microsoft Word, Excel and Outlook

Training Provided:
 By the end of the internship student should:
 Be able to identify and understand the symptoms, diagnoses, treatment and needs of adults with severe and persistent mental illness, particularly schizophrenia and schizoaffective disorder.
 Be familiar with community resources available to adults with mental illness
 Understand how to develop therapeutic rapport with a client.
 Understand the daily routine involved in providing support to clients.
 Learn the basics of the Trauma Informed approach to care.
 Develop a group protocol and lead a group.
 Learn documentation procedures for tracking clients' progress towards goals.
 Become a primary support person for clients on their caseload.
 Identify and become familiar with 10 medications commonly prescribed for this population.
 Complete an independent project in collaboration with your supervisor.

Type: Non-Exempt
Salary: $18.00/hour

SUMMARY: Provides mental health services as defined by CA State Department of Mental Health, including but not limited to: case management for residents including planning, services, monitoring, coordinating with other agencies and other activities as required. I. ESSENTIAL DUTIES AND RESPONSIBILITIES Must be able to perform each essential job function satisfactorily.
A. Direct Service and Case Management
1. In a culturally competent manner will provide services to all residents without regard to race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, and handicap or payment status.
2. Provide residents with counseling and training; assistance with daily living skills and managing psychiatric symptoms.
3. Assist in identifying and accessing medical, educational, prevocational, rehabilitative, social and other community services, specific to residents' needs, goals, and service plans.
4. Plan and implement cost effective and efficient service plans in conjunction with other staff, families and other service providers.
5. Initiate preventative measures and create intervention methods for clients or conditions that pose a threat of danger or crisis.
6. In conjunction with resident, family and providers prepare Annual Assessments
7. Inform individuals of their personal rights, responsibilities and Company's grievance procedures. Ensure services are compatible with residents' freedom of choice.
8. Transport residents to appointments, outings, etc. Maintain vehicle inspection logs.
9. Provide information to clients, families, other agencies and the community regarding health care issues including prevention, early detection and remedial care.
10. Other duties and responsibilities as assigned.

B. Teamwork and Collaboration
1. Maintain cooperative and professional working relationships and communications.
2. Give input to management on improving the quality of programs and assists in monitoring program effectiveness and conduct resident satisfaction surveys.

C. Reporting and Documentation
1. Mandated to report any "suspected abuse of dependent adults and elders" directly to law enforcement, Adult Protective Services, Ombudsman, or Community Care Licensing.
2. Monitor residents' progress and participation toward service plan goals, collect and report outcome data. Document coordination in progress notes and complete all documentation (E.g., annual assessments, service plans and progress notes) within required timelines.
3. Keep clear, accurate, and professional records in resident's files, staff logs, and other required communications.
4. Meet SCCMH and FSI guidelines for documentation and attend QIC as needed.
5. Meet individual billing expectations for Units of Service and work collaboratively to meet program requirements.

D. Activities (As Designated)
1. Help organize and conduct group treatment and recovery oriented structural activities.
2. Give capable residents the opportunity to participate in the planning, preparation, conduct, clean-up and critique of the activities.
3. Adhere to monthly schedule of planned activities.

II. QUALIFICATION REQUIREMENTS: Knowledge, skill, or ability required to perform the job.

B.A. or B.S. in social work or a related field; or two years experience working in mental health.
B. Knowledge
1. Requirements for providing care and supervision of residents.
2. Knowledge and conformity to all applicable HIPAA, OSHA, state and local laws, rules and regulations including Department of Social Services, Community Care Licensing, Title 22.
C. Language Skills
1. Read, speak, write and understand English well enough to follow and share the regulations, requirements and communications for providing care to residents.
2. Able to effectively present information and respond to questions.
D. Mathematical Skills
Calculate figures and amounts such as discounts, proportions, and percentages associated with completing the requirements of the position.
E. Reasoning Ability
1. Ability to carry out instructions furnished in written, oral, or diagram form.

4. TB clearance within the last 6 months and yearly thereafter. Pass a health screening.
5. Maintain a valid First-Aid certification.
6. May be required to serve on company committees.
7. Complete all annual training requirements as mandated by Community Care Licensing Division, Title 22 and Front St. policies.
8. Attend mandated meetings and trainings; fulfill all coursework and training hours required
9. Work in a safe and acceptable manner, following established safety procedures.

H. Physical Demands: Must be met to successfully perform the essential job functions.
1. Regularly required to talk, hear, and sit, stand for extended periods of time.
2. Frequently required to reach with hands and arms, stand, walk, stoop, and may occasionally be required to kneel, crouch, crawl or lift up to 50 pounds.

I. Work Environments: Employee may encounter while performing essential job functions.
1. Noise level is usually low to moderate.

3. The actions of clients may be unpredictable. Staff may be verbally assaulted and at times may be physically injured.
