Organization Description    Aspire Health’s Population Health division was created to improve the health and well-being in Monterey County. Owned by Montage Health and Salinas Valley Memorial Healthcare System, Population Health offers programs and services that help people live their best lives.
We collaborate with doctors and hospitals, health plans, patients, and community partners to promote wellness, improve care, and help lower healthcare costs for everyone.
We help people find solutions to overcome barriers to good health — financial, emotional behavioral, and clinical — so they can be more active, more independent, and more in control of their health.
We also help communities in our county address major health issues. We have a diabetes epidemic, and Population Health is leading the effort to combat this disease.
    
      Program Information    General Description of what students will be doing at this Site:
Interns will have the opportunity to assist with developing health education materials and resources, assist with patient management, to perform telephonic outreach to identify social determinants of health needs and connect people with appropriate health and community resources. Attend community events and support in home visits. This opportunity will provide diverse health promotion experience working with a variety of populations. The successful intern will be dynamic, self-directed, empathetic, multi-task oriented, a team player, and has a passion for helping others.
Key Responsibilities:
1. Provide support to all condition management and wellness initiatives
2. Assist with administrative duties including, but not limited to, manual entry of member information into database, sending health education materials, letters, and community resources, organization of the department, etc.
3. Interact with members telephonically and in-person to engage members in programs and services
4. Work collaboratively with Aspire Health’s multi-disciplinary team to meet member’s needs and provide excellent customer service.
