Dignity Health - Ambulatory Care Coordination

Program Information

The student will work with adult patients, assessing needs, addressing social determinants of health, developing care plans, providing education, motivational interviewing to help patients achieve goals and improve health related outcomes. Student will need to supply their own work phone. Computer and monitors will be provided.
