Stan Cares case management functions (engagement and support) o Intake meeting(s) and notations. o Follow up meetings and non-clinical case management functions. o Identify, explain and provide (campus and off campus/community) resource and referral information. Coordinate services with campus based partners and supportive customized success plans. Dean of Students and Student Conduct case management functions (engagement and support) o Pre-screening and intake meetings – initiate next steps, preparation for identifying and contacting appropriate departments and resources, determine campus based outcome and support. o Identify, explain and provide (campus and off campus/community) resource and referral information.
