Organization Description

Through the PACE model, WelbeHealth provides a full-service program that helps individuals age at home.

Program Information

• Conduct comprehensive psychosocial assessments • Lead care planning & coordination with IDT • Provide informal counseling and emotional support • Collaborate with licensed BH Clinicians on complex BH cases • Facilitate resource linkage & advocacy for community supports • Manage crisis intervention and participant safety issues • Provide end of life- Palliative Care Supports
