Program Information

Students will provide intensive case management services, completing individualized plans for clients, medical care coordination, attending doctor’s appointments, gathering medical records, advocating for clients etc., connecting to behavioral health and SUD services, go into schools, coordinate care etc.., connecting clients to medically supportive housing and related services, get medically tailored meals, get employment etc. Everyone must be able to manage their own schedule, manage the client’s schedules, carry a case load of 10-25 clients part-time, utilize electronic health record, be kind/courteous, maintain HIPPA confidentiality all regulatory requirements, write clear professional notes, work effectively independently and with a group, have a valid driver's license, have a professional presentation in appearance and speech, communicates well able to advocate for clients, able to utilize a laptop and a cell phone effectively, able to work with justice involved individuals (JI), comfortable working with the unhoused, comfortable working with the youth.

9am to 5pm regular hours, weekends for special events, nights for special events as they arise. Depending on client appointments, must speak to supervisor to come in earlier or to stay later.
