Organization Description

Del Puerto Health Care District is a California local government special district serving the west side of Stanislaus County through accessible, community-based healthcare services and regional partnerships. Operating Del Puerto Health Center and Patterson District Ambulance, DPHCD provides primary care, emergency medical transport, and expanding behavioral health access for rural and agricultural communities. Mission: To provide, promote, and partner in quality healthcare for all.

Program Information

Behavioral health assessment, brief intervention, case management, resource navigation, outreach, referrals, documentation, provider shadowing, care coordination, QI projects.

Health & Safety

HIPAA training provided; Clinic & District Orientation provided; CPR optional/training available
