Primary CounselorAt — Community Care Center

The purpose of this position is to provide general counseling, group therapy and casework services to assigned residents and their families. This position reports to Program Manager and program director.

Tasks:

• Prompt all residents for groups/activities, totaling all attendance weekly.
• Collect all data necessary to complete ABC forms and related documentation
• Provide timely and accurate monthly & quarterly, progress reports for clients on caseload.
• Participate in interdisciplinary care plan meetings.
• Directly responsible for the overall care of about 1-3 residents assigned as delegated.

Training:

Student will receive initial General employer orientation (mandatory) Pro-Act training (mandatory) daily meeting, weekly scheduled one-on-one mentor meeting, job shadowing, and overview/contextualization of assigned tasks.

Learning Outcomes:
