Program Information

Devoted Interdisciplinary Care Teams are made up of experienced hospice staff (Physicians, Nurses, MSW's Chaplains,Volunteers and Bereavement Coordinators) to meet the needs and identify the goals & create the plan of care. We offer pain and symptom management 24 hours a day, 7 days per week. Courage and compassion with steadfast support will be offered to all who inquire with OPTUM.
