Program Information Hart Community Homes provides residential care to at-risk, foster youth, between the ages of 12 - 18. Our Mission is to heal, educate and empower at-risk youth to become healthy, productive and self-sufficient adults in the community. We accomplish this by providing an integrated program of specialized and intensive services, supports, and treatment.
