Organization Description

This site is for Providence outpatient offices only (NOT hospitals). Faculty, please see list of approved outpatient offices and contacts posted as a log entry to the Parent of this group, or contact the Center for Community Based Learning at ccbl@humboldt.edu for more information.

Program Information

This site is approved for Shadowing/Observation activities ONLY.  Students, please make sure to list your specific clinic name/address in the Liability Release form when you complete it in S4 (and the Student Learning Plan if your faculty assigns it).
