Tracy Community Connections Center

Organization Description

We are a 501(c)3 nonprofit whose mission is to empower individuals to rebuild their lives through comprehensive support, resources, and community collaboration. We provide recuperative care, housing navigation, full-time case management, a mobile shower program, and a street outreach program while partnering with the city of Tracy, local agencies, hospitals, churches, and community organizations to care for the unhoused in Tracy.

Program Information

Engage with clients to identify and assess their needs in an empathetic, culturally, and ethically-appropriate manner; Utilize technology to track client interactions and make resource referrals; Intervene with clients and advocate on their behalf to obtain services such as housing, employment, and social services; Collaborate with clients in creating and committing to a plan for obtaining life skills and goals such as housing, employment, and self-sufficiency; Build relationships with clients and partner agencies in order to provide relevant and appropriate services.
