Program Information

At Freedom Home Health and Hospice Care, we provide professional, affordable, and caring in home living assistance to seniors throughout Yuba City, Marysville and Northern California. We are licensed by the State of California. We are dedicated to helping seniors continue to experience the richness of life in the comfort of their own homes. In order to do so, we provide high-quality and cost-conscious in-home care services to a wide variety of individuals in Northern California.

Educational opportunities for internships:

  • Interns will shadow Hospice team members to build an understanding of the scope of practice and funding streams for the 5 pillars of service provided under the Hospice umbrella - Nursing, Chaplaincy, Social Work, and Bereavement
  • Interns will be given educational opportunities to gain knowledge about and establish professional working relationships with patients, families/caregivers, and B&C/ALF/SNF residential care facilities
  • Interns will assist MSWs with assessing patients and caregivers for emotional and psychosocial status related to the patient's terminal illness and dying process 
  • Interns will learn how to address psychosocial needs and assist with providing support services and/or resources i.e., funeral planning and arrangements, advance health care directives, power of attorney, financial resources, In-Home Supportive Services, Medi-cal, etc. 
  • Interns will be introduced to and encouraged to collaborate with community liaisons, families, and community agencies in their common goal to serve Hospice patients
  • Interns will participate in interdisciplinary group meetings to assist the physician and other team members to recognize and understand the social/mental stressors and/or disorders that may exacerbate symptoms related to terminal illness. Interns may also contribute by sharing their observations, documentation, and collaborative insights with the clinical team related to the patient's plan of care
  • Interns will participate in program evaluation and development meetings, recruitment of volunteers, and/or research projects to improve their skills, improve agency outcomes, and to increase their knowledge of the many agencies who work together to serve the aging community 

Interns need to be comfortable working independently, in small groups, and with community partners. Flexibility and multitasking are essential to meeting patient's evolving needs. Empathy and compassion are necessary, as is the ability to self-reflect and be self-aware of personal biases and beliefs. Working with death and grief is a daily event. Interns need to anticipate experiencing a variety of emotions and/or empathy fatigue. A willingness to share and process feelings during supervision (and as often as needed) is required. 

Health & Safety

It is possible that interns will be exposed to communicable diseases, aggressive and/or combative patients, remote worksites, and a variety of home settings. Travel is required.
