Program Information At HEART, BSW practicum students will gain hands-on experience in a multidisciplinary behavioral health outreach and case management program serving individuals experiencing homelessness, housing instability, mental health concerns, substance use challenges, and other complex psychosocial needs. Students will learn in both field-based and shelter-based settings while working alongside experienced behavioral health professionals.
Throughout the practicum, students will develop foundational social work skills in trauma-informed and culturally responsive engagement, community outreach, case management, behavioral health screening, documentation, crisis intervention, advocacy, and resource coordination. They will also gain experience collaborating with multidisciplinary teams, including clinicians, peer specialists, medical providers, housing partners, community organizations, and county systems, while strengthening professional communication, ethical practice, and understanding of systems of care.
Students will observe how social workers navigate complex systems, advocate for client-centered care, and support individuals in accessing stabilization services and long-term resources. As students demonstrate readiness, they will progressively take on greater responsibilities through supervised practice, receiving ongoing mentorship and guidance from HEART staff to build confidence and competence in professional social work practice.
