Butte CARES Progam

Program Information
  • Butte CARES is a case management program serving individuals across Butte County who are experiencing homelessness or are at risk of homelessness.

  • The program primarily works with adults, but may also serve homeless youth or families when they are referred.

  • Services include housing navigation and Enhanced Care Management (ECM) for individuals with complex medical conditions who need support accessing and navigating healthcare.

  • After a client is placed in housing, the program provides 6–12 months of follow-up support to assist with tenancy stabilization and supportive services as clients transition from homelessness to permanent housing.

  • Students will learn about local systems of care related to homelessness and housing, including barriers to obtaining housing and root causes of homelessness.

  • Interns will gain experience working both in the office and in the field, including participating in direct homeless outreach in the community.

  • Interns will develop professional documentation skills, including completing case notes using the SOAP format and following standards set by DHCS and the local Managed Care Plan for the CalAIM program.

  • The program frequently collaborates with community-based organizations (CBOs) and other providers to ensure continuity of care for clients.

  • Interns will connect with these community partners and build professional networking relationships with organizations providing similar services.

  • Clients served by the program come from diverse backgrounds, including individuals who are justice-involved, and a willingness to work respectfully with all clients is essential.

  • The schedule is flexible, as daily workflow and appointments are coordinated by each case manager.

  • Work may take place between the Oroville and Chico offices, as well as in the community.

  • Company vehicles are provided to support travel between offices and for field-based work.
