Program Information

Psychosocial assessments; Identifying residents emotional, social, family, and environmental needs.

Supportive Interventions: Building Rapport and providing strengths-based support for grief, adjustment, loneliness, and loss of independance

Case management and discharge: Supporting transitions to home or other levels of care and coordinating services.

Linkage and brokerage: Connecting residents and families with healthcare, community, behavorial health, transportation, housing, hospice, and other supportive resources, including referrals and follow up.

Interdisciplinary collaboration: Participating in care plan meetings and working with nurses, therapists, social services, and other professionals.

Resident advocacy: Learning about resident rights, self-determination, confidentality, advance directives, and ethical issues.

Family support: Communicating with families, addressing concerns, and providing education and referrals. Documentation and professional practice; developing skills in progress notes, care plans, referrals, HIPAA, and professional boundaries. Supervision and professional development: Discusssing cases and ethical situations with an MSW to develop social work judgement and skills.
