MSW Case Manager InternAt — City Net- Sacramento County

Opportunity Summary 

As a student intern at City Net, you will have the opportunity to provide case management services to vulnerable populations such as our homeless community. You will learn and implement crisis interventions, goal planning, progress notes and get trained on the Homeless Management Information System (HMIS). HMIS allows you work with other partnerships in Sacramento County who also assist the unhoused. At City Net, you will learn about dual diagnoses (mental health and substances) and learn to apply Harm Reduction practices. You will also collaborate with clients on identifying treatment goals and create Individualized Housing Plan's for independent housing. Additionally, you will do outreach work in the community and work with other providers who also assist our unhoused participants. We are big on providing a safe space for everyone who walks through our door! If you are new to social work, come join our team!!!!

Opportunity Learning Outcomes 

Students will learn to apply social work practices when addressing crisis and emergencies with clients. Students will learn how to engage and disengage with someone who is affected by substance abuse or untreated mental health. Students will learn community resources to house our homeless communities by doing outreach work and learn how to collaborate with numerous partnerships. Students will understand the importance of a case plan and progress notes for funding purposes. Additionally, students will learn how to navigate the database of HMIS who will help them in future partnerships. Furthermore, students will learn that every member in the streets have a story, and all they need is an advocate like us who show empathy and give them a chance!

Opportunity Training 

Students will have the opportunity to shadow case managers and operations staff to learn the polices and positive impact of the program. Students will be trained on case management tools, safety goal planning, crisis interventions, harm reduction models, community engagement, and the Homeless Managment Information System (HMIS) database. This will allow students to learn about the tool homeless providers utilizes all over the county. Students will learn how to communicate effective with one of our most vulnerable communities in Sacramento County.
