Opportunity Summary 

At WelbeHealth, we provide Social Work students with immersive, hands-on training in interdisciplinary geriatric care settings serving older adults with complex medical and social needs. As a PACE organization, our model integrates primary care, behavioral health, rehabilitation, social services, and community-based supports under one coordinated team. Students don’t just observe — they actively participate in care planning, psychosocial assessments, family meetings, transitions of care, and interdisciplinary team discussions. They gain exposure to value-based care, regulatory frameworks, and whole-person approaches that address medical, behavioral, and social determinants of health. Our goal is to develop confident, systems-minded social workers who understand how to navigate complex care environments while maintaining deep participant-centered compassion. Through structured supervision, real-world casework, and collaboration across disciplines, students leave with practical skills, clinical insight, and a strong foundation in geriatric social work — ready to lead in the evolving landscape of integrated healthcare.

Opportunity Training 

Students will start their internship with a comprehensive orientation and onboarding, including but not limited to PACE overview, compliance training, EMR training, facility tour, IDT introductions, laptop and systems access, and personnel record requirements.

Additional risk specific to this opportunity 

Students will be briefed by our safety officer on all emergency preparedness and safety protocols.
