STAFF MEMBERS PROPOSED AS FIELD INSTRUCTORS:
Program Description:
The long-standing Internship Program includes an extensive Orientation to both FSA’s programs and to the older-adult network of services in San Francisco, including site visits and meetings with APS, SF General Hospital Inpatient Psychiatry, and Laguna Honda. The Internship Program is directed by Caroline Thomas, LCSW, a geriatric mental health specialist. Interns shadow team clinicians during Orientation, in order to observe different clinical styles and numerous clients and their individual situations.
|
Primary Issues that the Program Addresses:
Direct Service: Clinical Case Management is the Senior Division practice model; work includes comprehensive bio-psycho-social assessments of clients in office or home, and care planning in collaboration, when indicated, with an FSA psychiatrist or nurse practitioner.
|
Populations and Clientele Served:
Clients are all 60+ and for the most part have experienced life-long mental illness; they live in apartments, SROs in the Tenderloin and Mission districts, and in RCFEs. Interns have many opportunities to learn about assessment, diagnosis, and to practice and advance skills in Clinical Case Management.
|
Types of Services Offered by the Program:
Mental health services at FSA combine the best of therapeutic interventions with brokerage case management, crisis intervention (including 5150 evaluations), and extensive collaboration with other providers, including APS, IHHS, acute psychiatric and medical hospitals and, when possible, family members. The Senior Division offers a model of Transitional Care using group therapy in the Older Adult Day Support program; Interns are invited to co-facilitate one or more group therapy offerings.
|
Intern Assignments and Learning Opportunities:
Individual case-management, Group co-facilitation, attendance at all staff meetings, group supervision, individual supervision and attendance at the Monthly Geriatric Mental Health Training.
|
Other Specialized Training and Educational Opportunities:
Monthly Geriatric Mental Health Training
