Program Description:
The Institute on Aging (IoA) is a San Francisco-based nonprofit agency that has offered services to elders throughout the Bay Area in various iterations since before 1982. IoA’s mission is to enhance the quality of life for adults as they age by enabling them to maintain their health, wellbeing, independence, and participation in the community. IoA offers over a dozen medical and/or social service programs that serve older adults with a broad spectrum of backgrounds and needs.
This placement is with IoA’s Multipurpose Senior Services Program (MSSP). A Medi-Cal waiver program governed by the California Department of Aging (CDA), MSSP serves elders with serious health concerns that impair their ability to perform normal activities of daily living. These individuals are at risk of institutionalization in a nursing facility because they lack the support they need to remain safely in a community setting.
MSSP offers an alternative to institutionalization via long term, nonintensive care management that provides the resources needed to maintain a client’s safety at home. MSSP care management supports community living by fully assessing our clients’ needs, and then by monitoring chronic health conditions, coordinating medical care, ensuring adequate in-home support and equipment, purchasing needed services and items, and assisting clients and their caregivers and families with navigating the array of services available in their community. MSSP has nine social worker care managers who maintain caseloads of 48 clients; and two nurse care managers who carry small caseloads of their own and provide consultation and assessment for all MSSP clients.
Clientele Served:
MSSP is available at no cost to any San Francisco resident meeting all of the following criteria:
-Cal eligible Physical and/or cognitive impairment rendering him/her eligible for nursing home placement (assistance needed in 2 or more Activities of Daily Living), as certified by a licensed registered nurse
Direct services are offered in English, Spanish, Russian, Cantonese, Mandarin, Toishanese, Farsi, and Arabic. Services are also available via the use of a translator in several other languages, including Korean, Vietnamese, and Tagalog.
Issues Program Addresses:
To support the safety and independence of their medically fragile clients, MSSP care managers offer various interventions including direct provision of, referrals for, and/or purchases of the following:

Intern Assignments:
o The internship will be 2-3 days per week. o After training and orientation, the intern will co-case manage at least 3 but no more than 7 of his/her field instructor’s cases. The intern will conduct monthly contacts with clients via telephone and home visits, as well as initial psychosocial assessments and annual reassessments. The intern will develop and revise comprehensive care plans based on a) the client needs identified during monthly and annual assessments and care plan reviews, and b) the input of the MSSP multidisciplinary team in weekly team care planning conference. The intern will identify, coordinate, facilitate, and/or purchase needed services for clients. The intern will provide clinical justification for all services rendered, including ongoing care management services. The intern will coordinate independently with clients’ social support systems, providers, and community resources to provide the clinically appropriate resources necessary to maintain clients safely in the community. The intern will document all efforts, interactions, and purchases expended on a given caseload according to requirements explicated in the CDA MSSP Site Manual as well as those requirements listed in the IOA MSSP Policies and Procedures Manual. o The intern will receive weekly supervision from his/her Master’s-level field instructor, and monthly supervision with the MSSP clinical supervisor, an LCSW.
Intern Training & Educational Opportunities:
During the internship, students will have the opportunity to meet the managers of all departments at the Institute on Aging, and to work closely with other San Francisco/Bay Area agencies serving older adults. There are many topics available for research projects if needed. The student is eligible to attend all workshops presented by the IOA free of charge and will be notified as community presentations are announced.
Intern Benefits: (Include stipend, conferences, car, food, etc.)
No stipends, meals, or mileage reimbursements are offered. A car is helpful, but not a requirement of the placement. IoA makes of point of rewarding growth, and has a history of hiring outstanding interns upon completion of their degrees.
