Program Description:
Laguna Honda Hospital is the largest skilled nursing facility/rehab center in the country that serves 780 residents. The age range is from 18 years to over l00 years and population serves frail elders and adults with short and long-term disabling medical conditions. Residents have complex medical, mental health and substance use diagnoses and are primarily on Medi-cal. Laguna Honda offers a full range of services including medical and dental care, wellness center, language and dementia focused units, palliative care and HIV services.

Primary Issues that the Program Addresses:
Rehab from catastrophic injuries, treatment of newly diagnosed diseases, chronic illness and terminal illness and long term dementia care. Goal is to place residents at the right level of care and transition residents back to the community with a full range of services.

Populations and Clientele Served:
Short and long-term residents with disabilities, frail elders and homeless populations.

Types of Services Offered by the Program:
Complete range of medical and psychosocial support services. Units are arranged according to services and include an acute and SNF rehab center, palliative care, HIV positive, dementia, chronic care and Asian and Latino focused units.

Intern Assignments and Learning Opportunities:
Interns are part of a 20 member Social Services department with many years of experience in medical social work. Interns after a 3 week orientation with site visits to the community, will take on a caseload of 5-7 residents with varied medical diagnoses and cultural considerations. Interns will learn the full range of medical social work including intake and assessment, counseling, discharge planning, resource engagement and care planning and documentation.

Other Specialized Training and Educational Opportunities:
Interns take part in community training throughout the year that include reporting of elder abuse, working with LGBTQ residents, understanding competence and capacity, treatment of depression and anxiety, etc.
