Program Description:
Department of Care Coordination at Sutter Santa Rosa provides discharge planning services and social work support for patients/families with psychosocial issues that may impact their safe and timely discharge from our hospital. We are a team of nurse case managers and medical social workers who are committed to ensuring that every patient has a safe discharge plan from our hospital to return home or a higher level of care if needed.

Intern Assignments and Learning Opportunities:
Intern will be assigned to all hospital units (Emergency, Medical/Surgical, Intensive Care Unit (ICU), Neonatal ICU, Maternity, Outpatient Care Unit to conduct psychosocial assessments, offer emotional support, and provide community resources.

Other Specialized Training and Educational Opportunities:
Care Coordination Academy (mandatory orientation/training) to learn about medical social work.
