Program Description:

     Northern California Health Care System serves an area consisting of more than 377,700 veterans dispersed over a wide geographic area comprised of approximately 40,000 square miles and seventeen counties U.S. 

     Social Work Services provided by our agency include Ambulatory Care Medical Social Work, Inpatient Medicine Surgery Social Work, Community-Based Skilled Nursing Facility Program, Traumatic Brain Injury and Spinal Cord Injury Social Work, Community-Based Adult Day Health Care Program, Community-Based Homemaker/Home Health Aid Program, Home Based Primary Care Social Work Services, Health Care for Homeless Veterans Services, Homeless Outreach, Collaborative Initiative to End Chronic Homelessness (CICH), Grant & Per Diem Transitional Housing Programs, Chronically Mentally Ill Special Needs Homelessness Program, Post-Traumatic Stress Disorder Outpatient Treatment Program, and Mental Health Services.

Primary Issues that the Program Addresses:

Psychosocial adjustment of Veterans enrolled in VA healthcare.

Populations and Clientele Served:

Disadvantaged/oppressed clients served include:  Vietnam Veterans, Homeless Veterans, Female Veterans, Impoverished Veterans, Geriatric Veterans, Blind Veterans, Mentally Ill Veterans, Permanently Disabled Veterans and Minority Veterans.   Social Work in the VA has a long history beginning in 1926 with 36 social workers.  Today the Veterans Health Administration employs over 12,000 Master’s prepared social workers.

Primary duties:  Medical Social Work Services are provided to Veterans being seen in Primary Care.  Clientele are veterans and their families including:  Vietnam veterans, combat veterans, homeless veterans, female veterans, impoverished veterans, geriatric veterans, veterans of color, mentally ill veterans, dying veterans, physically ill veterans, spinal cord injured veterans, blind veterans, and minority group veterans.

Student social work activities:  Social work students will provide case management, psychosocial assessments and treatment planning to address psychosocial issues/needs such as:  homelessness, aging, limited resources, poverty, psychological trauma, disabilities, mental illness, medical issues, advance care planning, dementia, death and dying, lack of social supports, disenfranchisement, housing difficulties, spinal cord injury, post-traumatic stress disorder, family problems, substance abuse, economic problems, social maladjustment, acculturation barriers, legal issues, caregiver issues, system issues, and placement.  

VA Homeless HUD-VASH Program

     Primary duties involve providing case management to chronically homeless Veterans that recently have been approved for Section 8 vouchers through the program.  Case management includes finding housing for Veterans and relocating resources to ensure that they maintain their housing.  Case Manager utilizes different modalities; which includes Housing First Model, harm reduction, motivational interviewing, etc.     Other case management activities include initial admission assessment and intake process, crisis intervention and de-escalation of environmental distress, care coordination, coaching, brief solution therapies and assisting in normalizing environment for veterans who maintain independent living environments, coordinating services with other providers at the VA medical and mental health clinics, and referrals to services outside the VA.

Student social work activities:  case management, coordination of services, initial admission and intake process, referrals to services in the community, staff meeting participation, treatment plan development, documentation and charting, clinical consultation, case conferences, and participation in group support for Veterans, participation in the development of Cognitive Behavioral Therapy for HUD VASH recipients.

Therapies and evidence based practices that intern would be exposed to:  Cognitive Behavioral Therapies for relapse prevention, brief solution focus modalities with the goal for assisting veterans maintain stability.

VA Oakland Outpatient Behavioral Health Clinic

The clinic provides daily drop-in services, mental health and substance abuse treatment, clinical case management, individual therapy, homeless services, evidence-based PTSD treatment,  medication management and psychosocial and substance use support groups.

        Student intern social work activities:  psychosocial intakes, manage a small housing caseload, work collaboratively on an interdisciplinary team, possible group or individual therapy to assist homeless adults or those transitioning from homelessness, receive drop-in clients, work with high service utilizers, participate with interdisciplinary team, and develop care plans.

        Therapies and evidence based practices:  clinical case management, harm reduction, CBT, self-determination theory and practice, and wellness approaches for healing trauma.
