Program Description:

The Community Care Settings Pilot program (CCSP) assists Health Plan of San Mateo County members to transition out of nursing facilities and back to living independently in the community. CCSP also provides services to individuals living in the community, or those who are in acute care settings, that are at imminent risk of institutionalization. CCSP staff work to ensure that individuals are connected to all available community resources, stable housing, and appropriate healthcare services to support their ability to live at home safely.

Populations and Clientele Served:

Medical Eligible adults who are at least 18+ years old and aged persons with disabilities, mental health issues, substance abuse, and homelessness Hx.

Types of Services Offered by the Program:

Intensive case management, including referring individuals to all available community resources, stable housing, and appropriate healthcare services to support their ability to live at home safely. 

 

Intern Assignments and Learning Opportunities:

The intern will co-case manage at least 3 bu no more than 8 of his/her field instructor’s cases.

The intern will conduct monthly contact with clients via telephone and home visits, as well as initial psychosocial assessments and annual reassessments. The intern will develop and revise comprehensive care plans based on a) the client needs identified during monthly and annual assesses and care plan reviews.

The intern will identify, coordinate, facilitated, and/or purchase needed services for clients. The intern will provide clinical justification for all services rendered, including ongoing care management services.

The intern will coordinate independently with clients’ social support systems, providers, and community resources to provide the clinically appropriate resources necessary to maintain clients safely in the community.

The intern will document all efforts, interaction, and purchases expended on a given caseload according to requirements explicated in the CCSP Site Manual.

The intern will receive weekly supervision from his/her Master’s level field instructor, and monthly supervision with a CCSP Clinical Supervisor or Director.

 

Other Specialized Training and Educational Opportunities:

During the internship, students will have the opportunity to meet the managers of all IOA departments and work closely with other San Mateo Agencies. There are many topics available for research projects if needed. The student is eligible to attend all workshops presented by the IOA at least once per a month free of charge and will be notified as community presentation are announced.
