Organization Description The mission of Community Assessment, Planning, Education and Evaluation (CAPE) is to support the work of the Alameda County Public Health Department with timely, accurate, user-friendly information for public health surveilance, policy development, program guidance, and community development. This unit does epidemiology and community capacity building activities. Services of CAPE include: collecting, analyzing, and sharing information about health conditions, risks, and resources for county residents, helping public health departments and community groups to identify strategies for improving health, responding to requests for data on a wide range of health and social issues, helping public health programs and community groups evaluate whether they are meeting their objectives and improving community health, assisting in development of health-related local policy.
Program Information Your Kidneys and You – an education program to raise awareness among the 73 million Americans at risk for kidney disease. This program consists of a short presentation which includes educational materials and follow up survey given to community groups to raise awareness about kidneys, kidney disease risk factors, and how to protect your kidneys through lifestyle and self-management. NKF staff and interns do these presentations as well as train volunteers to find groups to present to as well. KEEP Healthy – This is an assessment and education program done in the community to help educate people on their risk factors for kidney disease, offer nutritional education, a clinical consult (MD or NP), and share resources and information on what tests to ask their care providers for that assess kidney function. If participants do not have a medical home, then education resources on how to find one are provided as well. A follow up survey is sent six weeks after participation. Bay Area Kidney Walks (Silicon Valley, East Bay & San Francisco) The Kidney Walk is the nation’s largest walk to fight kidney disease. The three Bay Area walks are among the 100 events nationwide that raise awareness, and fund patient and community programs that educate and support patients, their families, and those at risk.
