Program Information General Description of what students will be doing at this Site:
Student will participate as an intern with Community Health Partnership's Breast and Cervical Cancer Screening programs, Every Woman Counts (EWC) and the Community Mammography Access Program (CMAP). Intern will provide health education and patient navigation services to individuals' rarely or never screened for breast and cervical cancer screening in Santa Clara, San Benito, Monterey and Santa Cruz Counties. Intern will work with EWC's Lead Health Educator and Clinical Coordinator to complete the EWC Region 7 Community Needs Assessment to research and identify areas of need for EWC programming and services.
Key Responsibilities:
Intern will provide culturally competent breast and cervical cancer screening health education and patient navigation services to rarely or never screened individuals in Santa Clara, Santa Cruz, Monterey and San Benito Counties. Intern will provide assistance to individuals in need of completing a breast or cervical cancer screening by supporting individuals to navigate health care systems to complete recommended cancer screening services at no or low costs. Student will conduct follow-up communication with program staff and patients. Student will accurately collect and report outreach, education and patient navigation data. Intern will participate in program assessment and planning activities, including conducting research to identify breast and cervical cancer incidence rates, cancer screening rates, and demographics of populations residing in counties served by the EWC program. Intern will work with EWC team to develop a program implementation strategy to identify populations and geographic areas that may be benefit from EWC services in FY26 (informed by research conducted by student intern).
Desired Intern Characteristics:
Must reside in and/or share experience, languages and/or culture of the community served. Able to read, write and speak Spanish and or Vietnamese. Demonstrate public speaking and facilitation skills. Strong planning and organizational skills. Ability to work effectively both independently and on a team. Basic computer skills including email, Microsoft software, Google suite, and Zoom applications. 1-2 years of experience in community health, health services and/or equivalent education or skills, preferred.
Knowledge in data collection, preferred. Ability to work flexible schedules, including evenings and some weekends. Valid California driver’s license, access to a vehicle, and proof of liability insurance. Must be willing to travel throughout Santa Clara, Monterey, Santa Cruz and San Benito Counties.
Special Considerations:
Intern should be available to conduct on-site patient navigation and health education services at community health centers and or partner non-profit agencies throughout any of the four counties in Santa Clara, Monterey, Santa Cruz and San Benito Counties.
Interprofessional opportunities available for the student:
Student will work with a variety of public health and non-profit staff at Community Health Partnership, including Health Educators, Community Health Workers, Clinical Coordinator (RN candidates), and non-profit leadership including Director of Population health and Director of Community Programs. Student will participate in programmatic staff/team meetings to ensure collaboration and communication with staff for completion of projects. Student will receive training from Site Supervisor regarding delivering patient navigation services and providing health education.
