Program Information Our organization can provide professional-level learning opportunities that prepare social work practicum students for both direct service and systems-level social work practice within a rural public health and community services setting. Students would work alongside multidisciplinary teams serving vulnerable populations through programs focused on health equity, care coordination, prevention, and community wellness. Students would gain exposure to Enhanced Care Management (ECM), Medi-Cal service coordination, Community Health Worker (CHW/P/R) programs, youth prevention initiatives, and older adult services. Learning opportunities may include client outreach, resource navigation, care coordination, interdisciplinary collaboration, community engagement, and participation in trauma-informed service delivery practices. Students would also develop skills in professional documentation, communication, and ethical decision-making while working with diverse populations. In addition to direct practice experience, students would have opportunities to participate in macro-level social work activities related to program development and administration. This includes exposure to grant writing, project management, stakeholder collaboration, community assessments, outcome tracking, and evaluation of grant-funded initiatives such as ECM and the Youth Community Access Grant (YCAG) SOAR project. Students would gain insight into how public agencies develop, implement, and sustain programs designed to improve community health outcomes and strengthen local systems of care.
