Organization Description

Home Avenue ADHC is a day program providing a variety of health, therapeutic, and social services designed to serve the special needs of frail and elderly persons as well as adults who are disabled. Our therapeutic environment is designed to promote socialization and to maximize each individual’s potential for independence and self-esteem. Home Avenue ADHC also assists Family Members/Caregivers to receive respite and prevent burnout. Our goal is to restore or maintain an optimal level of health and to delay or prevent inappropriate institutionalization.
