Student Checklist - Students will need to provide proof of the following to Golden Valley prior to placement:
1. Background check. A completed background check order must be submitted to GVHC 90 days prior to start date, to be paid for by student.
2. Completed 9-panel drug screen (cocaine, amphetamines, opiates, methadone, propoxyphene, PCP (Phencyclidine), barbiturates, benzodiazepines, methaqualone. Needs to be completed 90 days prior to start date.
3. Current American Heart Association BLS CPR.
4. Training: student has been trained in OSHA safety measures, infection control precautions, and HIPAA. (GVHC will provide training student lacks.)
5. Proof of Student’s Vaccination/Immunity: Documentation of the following, or a signed declination:
i. Initial Hep B vaccine placed, or proof of laboratory titer;
ii. Two (2) MMR vaccine placed, or proof of laboratory titer;
iii. Two (2) Varicella vaccine placed, or proof of laboratory titer;
iv. TdAP vaccine within last 10 years, or proof of laboratory titer.
v. COVID-19 vaccinations and booster
vi. Flu vaccination within current flu season
6. TB test: PPD within the last six months, or chest x-ray within the last 4 year
- Background Check (e.g. fingerprinting/livescan, driving record)
- CPR Certification
- Driver's License
- Health Check/Screening (e.g. temperature screening, COVID-19 test)
- Must be 18 or older
- TB Test
- Vaccinations
- HIGH RISK: Health care services - Risk includes but not limited to: falling, needle sticks, medical exposure
- SITE REQUIREMENT: Site prefers (or has had contact with) students from a particular major or academic program, or with specific coursework completed (If so, please indicate above Special Instructions).
