Program Information

POSITION SUMMARY: The Social Work Intern provides care coordination services that span the continuum of care between the hospital and the community through facilitating the plan of care, transition planning, discharge planning and resource management. The Social Work Intern provides psychosocial interventions including psychosocial assessment, crisis intervention, short-term counseling, education and support groups, care coordination and resource referral to patients and families coping with medical treatment, illness, injury, and disability. The Social Work Intern collaborates with interdisciplinary staff, community-based providers, third party payers and representatives of community organizations and Shrine Centers. REPORTING RELATIONSHIP: This position works under the direct supervision of the Social Work Field Instructor. This position receives general supervision from the Care Coordination Manager, working within a framework of policies, procedures, and professional code of ethics.
