CommuniCare Health Centers is a Federally Qualified Health Center providing health care to those in need since 1972. CommuniCare provides comprehensive health care services delivered by a dedicated team of providers and support staff through our clinic sites and outreach programs. Our goal is to develop and maintain a long-term, healing partnership with each of our patients, while ensuring the best possible health outcomes. Serving communities throughout the Yolo County region, CommuniCare provides health services for one in every eight residents of the area. Our services include primary medical and dental health care, behavioral health services, substance use treatment, health education and support services.
Community Against Sexual Harm

Community Against Sexual Harm serves adult women that have been victims of commercial sexual exploitation. We operate a Drop-In Center 3 nights per week with an evening snack and facilitated groups, and we offer 1:1 peer mentoring sessions.

Sacramento County
- Auberry Park Apartments is located in South Sacramento County. It has 110 units with approximately 200 children. It is located in a low-income census tract, an area that is underserved by most social service programs. There is a Head Start Facility on-site, a small computer center, an active after school program. Community needs assessment and organizing would be a focus.
- Acacia Meadows Apartments is located in South Sacramento County. It is a 140 unit complex. There is a community room and a small computer center. Community Organizing and group work is a large focus for this community. There are approximately 200+ children living in this complex. The community is active with groups, activities, and a strong partnership with the local elementary school.
- Washington Square/Forrest Palms Both sites are located in the Alkali Flats/North Sacramento area. Washington Square is a 40 unit HUD Section 8 Project-Based Voucher community. There is an active youth program and opportunities for groups and community activities. Forrest Palms is a 40 unit, senior community (62+) that is also a HUD Section 8 Project-Based Voucher complex. The community has an active group and activity calendar, and strong resident participation. Field student can work both sites, focusing on youth, families and seniors. Transportation between the sites is required.
Yolo County
- Windmere Apartments/Walnut Terrace Both sites are located in a single-family neighborhood in east Davis, Yolo County. Windmere is a 106 unit multi-family site home to 120+ children. There is an active youth after school program, and the site offers opportunities for youth and family activities. Walnut Terrace is a 30 unit complex, for residents age 62+. The field student can work with both sites, focusing on youth, families and seniors.
- Hotel Woodland is an SRO, (Single Room Occupancy) Hotel, located in Woodland, Yolo County, which serves a diverse adult only population, who face a myriad of barriers to sustaining permanent housing, stable employment, and continuous mental health and health treatment. Field Students possessing familiarity with assessment and adult populations is highly preferred.
- Casa del Sol Mobile Home Park is a farmworker housing development with 157- mobile homes, located in Woodland, Yolo County. Thirty modular homes are designated as rentals. This multi-family site is home to 400+ children and has a large Spanish speaking population of residents. There is a community room and computer center. Casa has an active after school program and groups. This site offers opportunities for youth and family enrichment activities.
