Program Information

We provide protection to elders or disabled adults who are victims of abuse or neglect.
We provide caregiving help in the home for elders and disabled adults.
We work to prevent elders and disabled adults to fall into homelessness.

The MSW Intern Case Manager in APS and IHSS function as a geriatric care manager for frail, often socially isolated elders receiving services from Stanislaus County APS or IHSS. The MSW Intern Case Manager evaluates the potential client as a total person and identifies the functional limitations that impede independent living. He/she plans, provides and coordinates a wide range of services to assist clients in avoiding out of home placement and to remain living independently and safely as possible.
