Opportunity Summary 

Care Partners Program delivers whole-person, equity-driven services to older adults and caregivers across Alameda County. The program integrates interdisciplinary care coordination to support client independence, wellness, and system navigation, aligned with California Advancing and Innovating Medi-Cal (CalAIM) and Centers for Medicare & Medicaid Services (CMS) standards.

Internship Description
The Care Coordination Internship offers hands-on experience in case management, client engagement, and care coordination. Interns will work with an interdisciplinary team to support older adults and caregivers, gaining practical skills in public health and systems-level service delivery.

Intern Task & Responsibilities
• Conduct biopsychosocial assessments and develop person-centered care plans
• Track referrals and outcomes using the Unite Us platform
• Assist with data management, backup, and file organization
• Collaborate with healthcare, housing, behavioral health and aging services
• Contribute to graphic design and visual materials for client-facing and internal use
• Assist in designing and improving dashboards that expand existing reporting tools
• Provide bilingual support to clients and families (Cantonese, Mandarin, and Spanish)
• Assist with outreach activities, including weekend tabling events

Additional risk specific to this opportunity 

The Care Partners Program will provide comprehensive training to support the intern’s development in delivering whole-person, equity-centered services to older adults and their caregivers throughout Alameda County. Training will introduce the program’s interdisciplinary care coordination model, emphasizing client independence, wellness, and effective navigation of health and social service systems. The intern will gain familiarity with practices aligned with California Advancing and Innovating Medi-Cal (CalAIM) initiatives and Centers for Medicare & Medicaid Services (CMS) standards. Through structured learning, guided supervision, and hands-on experience, the intern will build skills in culturally responsive engagement, care planning, resource linkage, and collaborative service delivery within a diverse community setting.

How will you provide ongoing support and supervision to students?

The student intern will receive ongoing support from their supervisor through regular check-ins, guidance on assigned tasks, and timely feedback to promote learning and skill development. The supervisor will remain accessible for questions, provide clarification as needed, and ensure the intern feels supported in navigating their responsibilities. Students will also engage with Workforce Development & Grace Jordan who supports with secondary supervision and professional development.
